It's estimated that more than 7 000 hospital patients die each year because of drug errors, and many others suffer ill effects from being given the wrong drug or the wrong dosage Some hospitals are using bar codes attached to patients’ wristbands that allow hospital personnel who administer drugs to patients to electronically check to make sure the drug and dosage are appropriate Before administering a drug the doctor or nurse scans the bar code attached to the patient to see what drug is needed and when, and then the drug’s bar code is scanned to verity that the mediation is correct
But bar codes are not foolproof as a recent study of hospitals showed Nurses may develop a workaround that involves using photo copies of a group of patients bar codes which are then used to obtain drugs for then used to obtain drugs for the entire group. The nurse would then have a tray that may contain drugs of different dosages intended for different patients. At that point the bar code protection has been circumvented.
Why are bar codes being used in hospitals?

- Explanation of Benefits (EOB) or Remittance Advice – In many cases these terms are used interchangeably; however, they are different. The Explanation of Benefits is sent to a patient that explains how the insurance processed their services rendered, what was paid or not paid to the provider, and what the patient owes. In my time there, many animals passed away in their sleep or for unknown reasons. It was not uncommon. In these situations, our facility would offer the owners the service of an autopsy. An autopsy is a procedure in which the doctor would surgically open up the animal to check for any signs of what might have caused the animal's death. Mrs. Johnson, a client of ours, brought in her dog that had unfortunately passed away while she was at work. Her dog was only five years old, and the owners were not aware of any health problems. No one, including the doctor, could figure out what had caused the death of Mrs. Johns on's dog. Mrs. Johnson was asked if she would give her con- sent for the doctor to perform an autopsy on her dog so they might be able to answer the many questions surrounding his death.
