A community is defined as a locality-based entity, composed of systems of formal organizations reflecting societal institutions, informal groups, and aggregates that are interdependent and whose function or expressed intent is to meet a wide variety of collective needs (Stanhope & Lancaster, 2012). The yards are kept and I see an elderly couple outside doing yard work and raking leaves. I observe one neighbor hanging Christmas lights from the roof. The neighborhood has a mix of ethnicity. I see Hispanic, Caucasian and African populations. This family is Lutheran and attends a church a few blocks away. There is also a nearby Catholic church. The local hospital is about 1 mile away and I passed it on my way to their home. There are many doctors' offices and dental offices near the hospital. Many yards still have election signs still in the yards. There are not any billboards visible displaying any media. I do see some newspapers in driveways. This neighborhood has close access to the highway. The city of Chicago is about a forty five minute drive. The father says he takes the train to commute to the city for work in the city. He is a machine operator for a manufacturing plant in the city. This neighborhood is very culturally diverse and has a wide range of ages. There are both young and older families living in their neighborhood. Some aspects of the community that could affect resident's health are air pollution, aging populations, and noise disturbances. According to Health People 2020 leading health indicator for this population is environmental hazards. Some populations are disproportionately exposed to environmental hazards. Assessing a community is an integral part of Community Health Nursing because it leads to an understanding of a community’s overall health status, including its strengths and weaknesses. In turn, the assessment findings can be used to influence positive change in the community and improve the population’s health. One strategy used to assess a population’s health needs is performing a windshield survey. A windshield survey is typically conducted from a moving vehicle and includes a systematic observation of the general community. This paper includes the findings of a windshield survey I completed of Heber, AZ as well as my overall impression of the survey, and personal biases that I need to consider.

The boundaries of the windshield surveys consisted of the north to south areas located between The Gateway Park and 16th street, and west to east areas between 4th avenue and Arizona. This location is often referred to as “Historic Yuma” or “Old Yuma” and contains many historical buildings.
The town I chose to do my windshield survey on was Jamestown, NC, 27282. I chose Jamestown because this is where I grew up and this is where my grandparents live. Performing this windshield survey will allow me to observe all the things the town of Jamestown have to offer and to see what barriers may exist for a community health nurse trying to work with the residents of Jamestown. In my opinion a windshield observation is one of the most important tasks that a community nurse has to perform since this will allow them to know what is available for their patients.

A major influence on health in a community is the availability of services and opportunities within the neighborhood. Public health can be further influenced by the social environments. Accessibility to health care along with education and employment opportunities are fundamental factors to be considered (Riegelman & Kirkwood, 2015). These elements create both advantages and disadvantages for those living within the neighborhood. If we want to improve public health outcomes, we must consider and evaluate these along with many other contributing factors.
Many urban communities do not receive good quality of health and do not have access to health care services. This will cause individuals to be at high risk for several sicknesses and diseases. Some infectious diseases or illness of the immune system that they may be exposed to are asthma, diabetes, autoimmunity, Hepatitis C (HCV), HIV/AIDS, STIs, or tuberculosis. For example, “African Americans, American Indians, and Alaska Natives are twice likely to have diabetes as white individuals; diabetes rate among Hispanics are 1.5 times higher than those for whites” (National Conference of State Legislature, 2014). An additional example is “approximately 2 million Hispanics/Latinos in the United States have asthma, and Puerto Rican Americans have almost three times the asthma rate of the over all Hispanics” (NIH, 2013). Furthermore, urban communities deal with unemployment or live in low-income households. Even with families with a steady income they are still at a high risk for poor health. Geographic locations play a factor as well due to the environmental surroundings. Families that live close to factories or highly polluted areas are exposed to toxins that is not good for their health. One city that suffers through these environmental toxins are especially neighborhoods in New York. Chronic expose will lead to progressive worsening of health. The purpose of this community assessment was to identify the strengths and weaknesses using objective and subjective data on the community of North Judson, Indiana. This community assessment provides the foundation for improving and promoting the health of a community. The assessment identifies and describes factors that affect the health of a population, and factors that determine the availability of resources in the community to adequately address health concerns We used there vital statistics to determine the potential issues that this community faces and may face in the future. On the north end of the city, the boundary is economical with more expensive housing noted while exiting the inner city area. The west side of the city has a highway that separates the inner city residential area from an industrial park. The east side of the city becomes more rural while driving further out of the city. A business district that appears more modern is the boundary for the south side of the town.

For our windshield survey we decided to assess the city of Melbourne on a Friday afternoon, the day after summer vacation began. It was a nice sunny yet breezy afternoon, the children and some of their families were outside walking the streets, sitting on the porch and playing in the park. While assessing the city I noticed that the area was a low income community and consist of apartments as well as homes that are in need of cosmetic work.
