Approaches to Nursing
In this essay I will be discussing the processes which contribute to what I think “a good care plan” is. I will be writing about what I’ve seen in practice and what we used in our care plan in our poster presentation.
A good care plan according to NHS (2014), is a plan made to help provide you with support and outline all your needs and goals to help provide support to make sure these are all met. The care plan should consist of any health and social care support will be included to help the service user recover (NHS 2015).The service user who is being care planned for should be fully involved in the care planning process and any decisions made to help encourage independence (Health foundation 2013). We created a fictional character who was a 30 year old female with a supportive partner and a young daughter. She was suffering from depression which started round about when her daughter was 4 months old. She had taken over 3 impulsive overdoes in the past, of medication she had been storing away. This was her 4th admission to a hospital inpatient unit and we helped create a care plan we felt would provide the best possible care for her. We first decided to use the Orem model (1991) of self-care as the model is all about caring for yourself which our patient is capable of doing when better mentally. We used the model to help create goals for the patient so she could focus on meeting these goals independently and meet her needs. We also used the actives of living model (1983), to help the service user with encouragement to look at her routine and day to day activities and see how she can be supported by interventions and support from the team to improve her day to day life. We used this model as there were some concerns about the patient’s wellbeing due to her lack of oral intake and sleeping. We used the care plan to show how we would refer to a dietician and look at putting the patient on a fluid and diet intake chart. It was the first time I had seen a care plan lay out like this with a formulation that talked all about the patient and their likes and dislikes and what they are like a person. I felt this was a good example of person centred practice as every care plan was different and by reading the plan you got to know more about the patient as a person and you got more of an idea of how to care for them. It gave a clear outline of what the patient’s wishes were, the levels of interventions and support needed and their hobbies and interests. I felt this care plan was very personal and gave you an insight into what a typical day was like from their point of view (Alzheimer’s society 2015). The care plan included how to support the person best throughout the day such as how to get the ready for the morning and how to support the best to have a good day. I thought this was a good care plan for me, as I had never worked with the patients before and by reading their care plans I got more of an idea of how to provide the best possible support and what not to do when caring for the patient. I was unsure what model was used in the care plan but from learning on the course I believe it was APIE model. I will now talk about each patient needs as they all differ from each other. Nusrat Patel is 19 years old and has learning disability. This means Nusrat has difficulties in keeping knowledge and skills to the expected level of those the same age as her. Nusrat also has epilepsy which is neurological brain disorder when someone has epilepsy, it means they tend to have epileptic seizures, a seizure is a sudden attack of illness. Nusrat has left residential school to receive full time carer from her mum who has stopped working to care for Nusrat. At times this can be stressful so Nusrat attends the community centre on Tuesday and Thursday which allows Nusrat mother to have a break. Maria montanelli is 34 years primary school teacher who is much like Nusrat mother and takes care of her 96 years old mother who has dementia. Dementia is memory loss and difficulties with cognitive development. Being a primary care for her mother Maria feels she not performing at her best ability because of her lack of sleep which occurs when she assists her mother to the toilet several times. The last patient I would like to mention is Alice Fernandez she is 74 years old who recently lost her husband who had lung cancer. Alice doesn't use her pension the right way as she purchases many drinks as an alcoholic and has increased since her husband passed away. She has been prescribed antidepressant tablet by her G.P but made her lethargic this means she's become slow and sluggish.
It is important to follow the agreed care plan because it forms the basis of what is considered best interest for the individual, it is unique to them and details their needs and preferences. All care givers to the individual

In this report I will be investigating how care services meet the needs of individuals by firstly analysing the needs of an individual using care services. Then I will go on to explain the roles of the care planning process in identifying needs, and explain the features of a positive care practise. After explaining that, I will analysis positive care environments and evaluate how they meet needs before finally explaining the role of legislation in promoting a positive care environment.
Staff are required to make an entry in to an individuals care plan once in twelve hours. This entry is in the daily life and review and will contain details regarding medication administration, dietary and fluid intake, elimination, mobility, mood, behaviour exhibited and any changes or deteriation of the individual. There is also a requirement to record visits from doctors, nurses and other health proffessionals.
Within this essay, I will reflect and critically analyse an OSCE which has increased my awareness, or challenged my understanding, in assessing the holistic needs of a service user (John), referred by his GP, whilst incorporating a care plan using the Care Programme Approach (CPA). By utilising this programme and other sources of current literature, I hope to demonstrate my knowledge and understanding in relation to this skill as well as identifying areas with scope for learning.

This assignment will explore and critically evaluate the role of the registered nurse in the development of a plan of care that is patient centred. This will involve examining and critically analysing the chosen nursing model in a holistic assessment of the patient and the use of the nursing framework ASPIRE (Barrett, Wilson and Wollands, 2012).
The policy of person-centred care has become a mainstay of efforts to reform health care in the UK. Government policy is now built around this core concept, a major aspect of which is enabling individuals to participate in decision-making about their care at every level. By concentrating on the individual’s wishes and aspirations and placing them at the centre of a ‘planning circle’ the care team can assist with goal setting to ensure their best interests are maintained and the individual’s preferences are considered.
It is important to review care and support plans as people’s needs change. By including the person, their family then everyone knows what is happening and the family can help to monitor mood swings and behaviour. The individual and the family can express their views and preferences and any relevant risk assessments may be done with everyone involved. By monitoring the individual, a decision can be made as to whether the changes are effective and if the best care is being given to encourage independence and promote dignity.
It is necessary to involve the individual in the plan of care and support. Encourage the individual to make choices. This includes their needs, their culture, their means of communication, their likes and dislikes, wishes and feelings, advance directives, beliefs and values, involvement of their family and other professionals. This should be considered and documented. Also, there must be evaluation in assessing effectiveness in the plan of care.

The aim of this assignment is to critically discuss the nurse’s role in planning care. The case of care is a 60-year-old lady named Mary. The names in this assignment have been changed in order to remain confidentiality in line with the Nursing and midwifery Council (2015). Mary has a son and lives with her husband of 30 years. Mary was diagnosed with type one diabetes mellitus when she was 18 years old. Mary also has a leg ulcer which is effecting many aspects of her life. Although Mary has other health problems I will be mainly focusing on the leg ulcer.
