A basic overall assessment of Pasco county health concludes that the county is a moderately healthy community with chronic illness being a major issue. When comparing Pasco County In the area of Health outcome to the 67 other counties in Florida Pasco places 45th. The higher the score the worse the county is. (County Health, 2013) There are two categories that the rankings were broken into were health outcomes and health factors. Health outcome refers to mortality and morbidity. Health factors pertain to health risks including factors like alcohol use, access to care and education. With Health factors Pasco ranked 28th. The rankings vary by a large margin and lead to the conclusion that the county has relatively healthy.
What I can infer from the statistics found there is a major issue with chronic diseases and other health factors, which has the potential to lead to a very unhealthy community. The statistics shows that there is a higher diabetes rate in the county then the average for the state. When it comes to communicable disease, Pasco has below average contraction. An example would be Infection with HIV or AIDs, Pasco has around 2.4% while the state average is 5% (County Health, 2013). While this is a lower number, there are other areas where we are above the state average. An example of this is the smoking rate in Pasco County. Smoking is widely known for its horrendous effects on health, in Pasco there is a 26% of adults smoking while the state average is 18%
Cancer has been the leading cause of death for Volusia County since 2006, with heart disease at a close second. The county’s age-related death rate from cancer is 182.3 per 100,000 people as compared to the state’s total of 160.4, and the national average of 179.1 (Fl Dept of Health). Volusia also has a greater rate of adolescent cigarette smokers, the state average being 6.1%, and Volusia County being 8.3%. Volusia County’s adult smoker population also is greater than the state average, 23.3 vs. 17.2. Volusia residents suffer from heart disease greater than most other Florida counties. In 2012 there were 112.7 deaths per 100,000 people as apposed to the state total of 103.5 and the national total of 179.1. Another concern is with the growing obesity population in which comes with it increased risk for hypertension, heart disease, and many other diseases. Both the county and state totals are approximately 82% higher than the national rate. A group of Volusia County residents, called Healthy Volusia, has made the problem with obesity a top priority (Dept of Health).
Another strategy would involve utilizing data from existing public sources to assess the health and safety needs of the people of Utah County. The main elements of the collected data would involve the identification of the key health indicators and the corresponding risk factors. I would also collect information related to the impact of the social, economic and the environment on the health status of Utah inhabitants. The key sources of data for the assessment of needs of Utah populations would include US census bureau, healthy people 2020, environment public health tracking program, department of mental health and substance abuse and Utah hospital discharge database. Other sources of information for the need assessment would include the indicator based information system indicator for the public health department and Utah’s healthcare access survey (Niederdeppe, Bu, Borah, Kindig & Robert, 2008). The health status of the population in the United States is a national priority. The Healthy People 2020 outlines priorities and to improve the health of individuals and communities across the country. Healthy People 2020 is defined as a comprehensive set of national objectives for “improving the health of all Americans” (Healthy People 2020, n.d, para. 1). The Healthy people 2020, is aimed at improving the health of individuals and communities and envisions “a society in which all people live long, healthy lives” (Healthy People 2020, n.d, para 4). A short priority list was developed and contains twenty-six objectives knowns as Leading Health Indicators (LHI). The list of objectives was created to prioritize health issues that must be addressed and to find appropriate actions to take in order to improve the health of the population.

Environmental safety hazards found in Brazoria County include drinking water violations, severe housing problems, and driving alone to work (County Health Rankings & Roadmaps, 2015). The purpose of this paper is to provide insight on health disparities seen within DeKalb County. The residents of DeKalb County are diverse in race, ethnicity, and income. Since the county is so diverse there are many health disparities seen amongst its residents. Health disparities occur within a county when one group of a population has noticeably better or worse health than another group within the same population. These groups of people can differ by income, education, sex, race, location, and even sexual orientation. This Paper will discuss the health disparities that exist within DeKalb County, and explain why these disparities exist from a historical, political and social aspect. Furthermore, this paper will give background information on health outcomes from national, state, and local epidemiological perspective in order to support the claims that disparities actually exist within this county.
A community’s health can be established through a myriad of differing factors. Three specific factors that will help to determine the health of the county in which I live are community safety, disaster preparedness, as well as an analysis of the population. The community under review, Merced, is located in central California and made up of seventeen cities and communities whose individual populations do not exceed more than 75,000 and covers an area of approximately 1,972 square miles (United States Census Bureau, 2010) with a total population of approximately 255,793 people in 2010 (Merced County California, 2012).

A community health assessment is a fundamental instrument of public health practice. Its objective is to depict the health of the community, by presenting information on health standing, community health needs, resources, and epidemiologic and other studies of present local health problems. It seeks to recognize target populations that may be at augmented risk of poor health results and to increase a better understanding of their needs, as well as evaluates the larger community surroundings and how it relate to the health of people. It also identifies those areas where better information is desired, particularly information on health differences amid different subpopulations, quality of health care, and the incidence and severity of disabilities in the population. The Community Health Assessment is the foundation for all local public health development, giving the local health component the instance to recognize and network with key community leaders, businesses and concerned residents about health priorities and concerns. This information shapes the foundation of improving the health status of the community by way of a strategic plan (The Municipal Public Health Services Plan Community Health Assessment Guidance and Format, n.d.).
Pierce County has performed 4 MAPP assessments to determine how residents can achieve optimal health. Together, the 4 MAPP assessments will help the Pierce County health system devise a plan where residents can safely play, work and live together. Pierce County’s multitude of parks and recreation, and built environment is a noted asset in the CHSA and CTSA that are unfortunately underutilized by community residents. The Community Health Status Assessment (CHSA) confirms that there is a worsening trend in adult obesity and physical inactivity (countyhealthrankings.org, 2016). Due to the mere size of California each county is broken up and evaluated on its medical needs. Basic healthcare needs are available for everyone but due to the lack of accessibility funding was put into creating community health clinics in much of rural parts of the state. Defining this purpose was the diving board of serving the communities throughout California. Social and economic factors play a large role in my county because of the lack of education, language barriers and agriculture (seasonal work) seems to feed the poverty levels, which in turn are very high. Health behaviors are hard to break that include consuming fried and highly refined foods, smoking and sedentary activities leading to obesity are of great concern while costing tax payers millions in the long run.
According to Healthy people, health determinants is the range of personal, social, economic, and environment factors that influence health status.1 The overall health rank of Virginia health status is 21. Virginia has improved over the years but there is still room for improvement. The strengths of Virginia health status includes: low rate of drug deaths, low violent crime rate, and low percentage of children in poverty.2 The challenges of Virginia health status includes, low immunization coverage among children, low immunization coverage among teens, and high infant mortality rate.2 These challenges that Virginia faces can conclude the reason for the percentage of the overall health status not rising dramatically.

Essex County was officially established in 1682 by east jersey legislature. Essex County is 129.631 square miles. The Geopolitical communities are designed by natural or human made boundaries (Maurer and Smith, 2009). It holds community safer for the residents and the governing body.
Community health assessment is a process of systematic identifying and examining health indicators in a defined population. The purpose of this community health assessment is to gather and analyze information about health status, factors that can have an influence on community members’ health, and needs of the population within the target community. The collected data allow comparing the health status and needs of the residents in the Newmarket Health Centre (NHC) and residents in surrounding neighbourhood with the local health care services and resources available for them, which will help to identify health inequities among community members (Goodman, 2014).The main goal of this community health assessment is the strategic development of solutions to the identified health needs in a community.
