Case Conceptualization Client Description: 44-year-old, cisgender male, identifies as Cherokee Indian, previous military history. Name: Keyan Age: 44 Presenting Issues: Case description including presenting issue(s), referral source, identification of any personal, social, emotional, family/family of origin, relational, or academic/career issues. This description should include specific examples, incidents, and support from the literature. Cite references within the text as you describe the client (minimum of two paragraphs). Keyan is a 44-year-old cisgender male with a history of military service. Keyans’ current full time. employer sought counseling for him after he encountered problems with alcohol use resulting in missed work, being hung over at work, and visible bruising which as the result of physical aggression towards his spouse. Keyan enjoys a variety of activities, is socially active with the Cherokee Nation, and hangs out nearly every week. He also has enduring friendships. His mother, father, and a grandpa who, according to Keyan, has abstained from alcohol for decad es are among his ancestors. Keyan reports that he has engaged in drinking since his teen years with a recent increase to daily use impacting his relationship with spouse and work He frequently has a depressing and solemn expression and expresses worry about the future.He  mourns the passing of a longtime friend someone he had worked with in the military, someone h e had been socializing with, but who committed suicide, making it harder for him to sleep. He acknowledges that he drinks at home to relax and avoid conflict; but recently noticed this can lead to his increased aggression. He also disclosed that he had recently had a drunken argument with his wife. The loss of his companion and Keyan's alcoholism are critical aspects to look into.Reingle et al. ( 2014) claims that heavy drinking on a regular basis interferes with the neurotransmitters needed f or psychological wellbeing. Even though people may feel peaceful after a drink, alcohol affects their mental health and can c ause emotions of hopelessness and anxiety as well as make stress harder to handle. Since excessive drinking is linked to alcoholism, therapists who work with clients who abuse or  depend on alcohol may find it difficult to diagnose their patients' mental problems. Assessment: Discuss the process of assessing for and identifying additional issues/needs of the client. In this section, identify what you think might be going on with the client. According to Essau et al. (2018), anxiety and substance use disorder (SUD) are highly comor bid in childhood and emerging adulthood; recognizing and further assessing the alcohol use will allow for increased intervention utilization to support managing anxiety in a realistic way. The progression of Major Depressive Disorder (MDD) and anxiety disorders over the four life st ages examined by Essau et al. (2018) is a crucial additional aspect to consider. Keyan's circumstance presents a conflicting dilemma because substance misuse needs to be addr essed. 2.
