Geographical location plays a significant role in how state healthcare policies are put into action. National healthcare policies influence local and state regulations, while the development of local and state policies involves community engagement Healthcare policies are tailored to address the distinct healthcare needs, resources, and political landscapes of different states and regions. As each state and region possesses unique healthcare needs, resources, and political landscapes, their approaches to implementing healthcare legislation also vary accordingly in Rural areas. The decisions regarding state Medicaid expansion were based on political beliefs, population demographics, and regional differences. Research indicates that states in the southern and midwestern regions of the United States were less favored to expand Medicaid compared to those in the northeast and west. This modification is believed to be influenced by political elements, including the party affiliations of state governors and legislators. Also depending on the program, such as Medicare and Medicaid, administration and management occur at either the federal or state level. Medicare, for example, is a federal health insurance program providing coverage for hospital care and doctor visits for individuals aged 65 and older, those with disabilities, and individuals with specific medical conditions. National coverage determinations and exclusions are set by Medicare, while local coverage decisions based on medically necessary services in each region are determined by Medicare-processing entities operating within individual states (Brown, 2029) . Rural residents in geographically isolated communities in the U.S. face significant health disparities compared to their metropolitan counterparts. Implementing telehealth policies in rural areas presents distinct challenges such as
limited broadband access and inadequate resources for telehealth infrastructure. It's imperative that telehealth policies are customized to address the specific needs of rural communities, ensuring equitable access to healthcare comparable to urban areas ( Hall et al., 2021) . Challenges in accessing care, particularly among individuals with public insurance and in rural regions, have been observed. In essence, geographic location influences the implementation of federal or state healthcare policies, necessitating tailored approaches to accommodate the unique needs and resources of diverse regions ( Loftus et al., 2018).
