Purpose of the assessment
This assessment task is designed to evaluate student’s following skills and abilities:
Understanding the process of checking clients health status.
Knowledge of potential risk factors for common illnesses.
Knowledge of appropriate referral services.
Task instructions
Please read through the following case studies. Reflect on your learning and your own research to ensure your responses are satisfactory. You should consult a range of learning resources and other information such as handouts, textbooks, learner resources etc. Your responses must reflect your knowledge, skills, and application for this unit.
This is an individual assessment.
You must answer the question as per the client scenarios given in the case studies.

Task 1 Case study 1
Martha is an 83-year-old woman who has been cared by for her daughter in her daughter’s home over the last five years. Martha has dementia and is getting progressively more forgetful and confused. Martha’s daughter reports to you that Martha has been visiting the doctor more frequently due to deterioration in her overall health. Martha has not only been diagnosed with dementia, but she also has ongoing issues with her breathing. Her doctor has indicated that she may have chronic bronchitis; however, this requires an official diagnosis, something Martha has not yet agreed to. It has come time for Martha to be cared for by professionals as her daughter is unable to keep up with the medications and the monitoring Martha requires. Martha’s daughter is visibly upset during your meeting, stating that she feels like she is failing her mother, and she never wanted to put her in a home. In the meeting with Martha, Martha complains of a sore tooth.
Questions
1.
Identify how you can obtain further information to check Martha’s health status using your knowledge of the body systems. (50-70 words).
Access to Martha’s past medical and health history records will provide clearer information regarding her health status. This can be obtained with consent from Martha and her family. I would also obtain consent from my supervisor to ensure I have access to this material. This information could be obtained through a questionnaire or interview process, from a supervisor or doctor.
2. Describe any actual and potential problems for Martha. (50-70 words)
As Martha has been diagnosed with dementia, she will become increasingly forgetful and day to day tasks will become more difficult. One actual problem for Martha would be an increase in confusion, which would lead to falls and accidents, resulting in physical injury. A potential problem could be the emotional distress of moving into a new unfamiliar environment due to the
transition into professional care.

What interventions may be suitable in this situation to improve Martha’s health status? (40-60 words)
To ensure Martha is living within the healthiest state possible it is important to review her physical, psychological, and social health. This will help create a clearer picture of her overall health status. As she is already making frequent trips to the doctor, a medical assessment may Page | 2

not be necessary as it may already be apparent. 4. Identify the people you could clarify the significance of Martha’s health issues with if you were uncertain or it was outside the scope of your role. (10-20 words)
I would refer Martha to a Dentist and a geriatrician or aged care worker. 5. From the list of people identified in Question 4 complete the table below identifying two and clarifying their roles, responsibilities, and limitations in confirming Martha’s health status.

If left untreated bronchitis can lead to pneumonia and damage the cardiovascular system, subsequently leading to chronic obstructive pulmonary disease. If Martha’s dementia is left untreated her overall quality of life will deteriorate in all aspects, such as her social, physical, and mental wellbeing. Martha and her daughter have made the decision to keep Martha in the family home but utilise the support of professionals to come in and assist in caring for Martha. To make sure this is possible Martha needs to have a full examination and assessment of her overall health status so that the correct support can be provided for Martha and her daughter. You will be required to arrange and participate in this assessment process.
7. Provide the list of services Martha will be referred to and the role and responsibility of each will play in confirming her health status. (70-90 words)

1. A Pulmonologist – a physician who specializes in the respiratory system. To treat chronic bronchitis.
2. Dentist - a dentist to look at the sore tooth and treat it so the pain can be minimized.

In home aged care worker – to provide personal care like showering, oral hygeine, dressing, cooking, monitoring mental health and wellbeing, and helping with household tasks like cleaning or laundry for Martha. 4. Task 2 - Case study 2
You are a community support worker for the local council that provides in-home care for the elderly where required. You have just met with Paul for the first time and begin to gather some information about Paul and his health status. Paul tells you that he has recently been diagnosed with diabetes and he is concerned this will impact the level of independence he has. Paul is in remission from cancer and so far, all his tests are looking positive for staying cancer free. Paul currently lives on his own although discloses to you that he is finding it difficult to keep up with the bills of running his home. Paul has two children that live a few hours away and only come to visit every six or so weeks. Paul was reluctant to meet with you at first however he tells you that he is glad that he did and feels comfortable talking to you.
Questions
1. Describe how you could identify the cause of Paul’s diabetes, including family history and environmental factors. (40-50 words)
Family history and genetics are a strong determinant contributing to Paul developing diabetes. Genetic inheritance contributes to the likelihood of developing certain illnesses. Coupled with the quality of conditions of his physical environment, clean air, safe water, healthy workplace. If these aren’t aligned this could contribute to the cause.
2. Confirm which body systems Paul’s diabetes may affect and how this could impact Paul’s overall health. (50-70 words)
Diabetes will affect majority of Paul’s major organs and organ systems. Namely, the cardiovascular system, the nervous system, the immune system, the urinary system, the integumentary and digestive system. Overall, diabetes will affect Paul’s overall health as his mental health, emotional and social health will be challenged in day-to-day life. His ability to engage in physical activity will also be compromised. 3. Given Paul’s recent diabetes diagnosis and you not being a diabetes specialist, what could you do to
support Paul in learning more about this disease? (50-70 words)
There are plenty of options to aid Paul through his journey. Firstly, attending specialist appointments with Paul would be a great start in providing physical and emotional support, plus, this will help in getting a better understanding of the disease. Other options could include encouraging better lifestyle choices, for example, going on walks together, or preparing nutritious food. Page | 5

1. Describe the five (5) different structural levels of the human body system. (200-220 words).
To describe the different structural levels of the human body it is important to understand the basic architecture; how the smaller components constitute the larger structures. Structurally, the
human body is set up into fundamental levels of organisation, increasing in complexity. These levels of structural organisation start with the smallest unit being chemical building blocks, up to an intricate organ system. The smallest level is a chemical building block which is formed of subatomic particles, atoms, and molecules. A molecule is the chemical building block of all body structures. A cell is the first level, this is the basic structural and functional unit of a living organism within the body system. Secondly, tissues are a group of cells that are formed to work together to perform a specific function. The third level in the human body system is organs. Typically, two or more tissues combine to form an organ, those organs carry out many various and specific physiological functions. The second last level is when organs form together and make an organ system. Organ systems are often interrelated and work together to carry out complex physiological functions. Such as pumping blood, and processing and utilizing nutrients, for the body to operate optimally. Lastly, organ systems form to combine an organism. This is our
highest level of the human body system.
2. Describe each of the life processes displayed in the picture below. (150-200 words).
All living organisms have specific characteristics that distinguish them from non-living organisms. The basic processes of life are interrelated and work together to maintain life and overall wellbeing.
